When you submit a request, Karelink Health may collect your name, phone number, email address, preferred contact method, appointment timing, pickup information, destination, and any optional details you provide. We also collect limited technical information needed for security, fraud prevention, and consent records, such as the submission time and internet address.

Please do not submit diagnoses, medical records, Medicaid identification numbers, or other private medical information through the website form.

How we use information

We use submitted information to respond to your request, review ride availability, coordinate transportation, send service-related email, and—only when you choose the SMS option—send transactional text updates about that request and ride.

How information is shared

We may provide limited information to service providers that help us operate the website, deliver messages, secure the service, or arrange transportation. They may use it only to provide those services on our behalf.

We do not sell personal information. Mobile numbers, SMS consent records, and SMS opt-in data are not shared with third parties or affiliates for marketing or promotional purposes.

Text-message choices

SMS participation is optional and is not a condition of receiving service. You can reply STOP to any Karelink Health text to opt out. Reply HELP for assistance or call 312-838-2100. See our SMS Terms for program details.

Security and retention

We use reasonable administrative and technical safeguards intended to protect submitted information. No internet transmission or storage system is completely secure. We retain information only for as long as reasonably necessary to provide service, document communications, meet legal obligations, and protect the service.
